Required Qualifications
• Associate degree or higher
• 1+ years of experience with Electronic Medical Record (EMR) or Other EMR or TruChart EHR
• Valid Registered Nurse (RN) certification/license in Pennsylvania (PA)
• Valid American Heart Association Basic Life Support (AHA BLS) or American Heart Association Advanced Cardiac Life Support (AHA ACLS) certifications/licenses
• 1+ years of experience with RN
• Do you have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all 3)?
• Have you spoke with a Relode Recruiter about this role (via phone)?
• Are you okay with the required travel for this position?
• Have you applied to or worked for Residential Healthcare Group anytime in the last 6 months?

Overview
Our client is seeking a Home Health Registered Nurse Case Manager to join their extraordinary care team. This position supports patients in Shippensburg, PA and surrounding areas. Key Responsibilities
● Lead and collaborate to provide management and delivery of patient care plans
● Support patients in Shippensburg, PA and surrounding areas
● Eligible for a $15K first year success bonus to reward the accomplishment of key achievements during the first year

Schedule and Shift Details
● Mon-Fri 8am-5pm; on-call rotation is only 1 weekend every 4-8 weeks depending on staffing. When it is their scheduled weekend, they are on-call Saturday & Sunday, 8am-5pm only for SOC visits. They do not do on-call during the week or after hours.
