Advanced Practice Nurse, Primary Care Center

Visiting Nurse Association of Central Jersey Community Health Center (VNACJ CHC), a federally qualified health center in Monmouth County, dedicated to offering accessible, high-quality healthcare to New Jersey residents. Our comprehensive services span preventive care, primary care, wellness support, mental health counseling, prenatal care, and more—all delivered in a compassionate, community-focused environment. Position Summary:

The Advanced Practice Nurse (APN) provides high‑quality primary care to patients of all ages in our Keyport, NJ community. This full‑time role focuses on evaluating and managing acute and chronic conditions, delivering preventive care and education, and supporting patients’ long‑term wellness. The APN works closely with a dedicated clinical team to deliver compassionate, patient‑centered care that strengthens the health of our community.

Principal Responsibilities:

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Provide palliative care or hospice face‑to‑face evaluation services within established productivity guidelines (if applicable to the role)
  • Deliver patient and family education to support understanding of diagnoses, treatment plans, and preventive health strategies
  • Monitor the quality, safety, and appropriateness of services provided
  • Identify opportunities for new services or programs aligned with the organization’s mission and community needs
  • Adhere to infection control, safety policies, and all regulatory guidelines governing primary care practice
  • Provide supervision and support to staff in alignment with organizational leadership standards


Job Competencies & Minimum Qualifications:

  • Active New Jersey Advanced Practice Nurse (APN) license
  • Master’s degree in Nursing from an NLN‑accredited program
  • Certification as a Family Nurse Practitioner (FNP) with prescriptive authority; active CDS and DEA registrations
  • Minimum of 2 years of primary care or ambulatory care experience
  • Business or management experience preferred, with demonstrated leadership abilities
  • Reliable transportation, including a valid driver’s license and personal vehicle


Working Conditions/Physical Demand: Working indoors. Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing and walking. Client/customer contact

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $120,000 to USD $170,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
